Proof of Identity: (one of the following)

  • State Issued Photo ID

  • Valid Driver’s License

  • State Issued ID Card

  • Military ID Card

  • U.S. Passport

  • Social Security Card (if number is not on ID supplied)

You must provide complete and correct information about the agency to receive results. The following items are required:

  • Address Must Include:

    • Address Number

    • Street Name

    • City

    • State

    • Zip Code

  • Agency Name

  • Contact Person

  • YOU MUST KNOW THE REASON CODE, provided by the employer for the fingerprinting, as it is required by law.

Requirements

OUR OFFICE DOES NOT ACCEPT CASH.  PLEASE BRING A CHECK OR MONEY ORDER. WE DO HAVE A CARD MACHINE, HOWEVER, IT HAS A $3.95 CONVENIENCE FEE.
